The Hope Center for Rehabilitation’s case management team is tasked with the important duty of coordinating, implementing and reviewing each patient’s treatment plan. Because they are so involved in each aspect of a client’s recovery, from medication to therapy and beyond, case managers are an indispensable asset to each patient’s overall recovery.

At the Hope Center, we take great care to provide a highly individualized treatment program for each addict and alcoholic. Every one of our patients works with their assigned case manager on an individual basis in order to assess and get what they need out of their treatment.

• The tasks of a case manager broadly include:
• Assessing a patient’s physical and mental health needs
• Developing a program to provide for those needs
• Implementing this program at each phase of rehabilitation
• Reviewing and updating this program on a regular basis

At the Hope Center, we recognize that addiction and alcoholism extend far past the use of drugs and alcohol. They are life conditions, and they often warrant services that go beyond the scope of addiction treatment. That is why case management works not only with our in-house therapists, doctors and behavioral technicians, but also with law enforcement, the judicial system, and any other area in which an addict needs help. Most addicts struggle with legal problems, debt, and/or employment.

Case management thus necessitates communication and guidance in each of these areas, as needed on an individual basis.

Why Case Management is Important

Case managers provide a single point of contact with healthcare and social services systems. They prove an exceedingly important part of recovery both for those who have been struggling to maintain sobriety for extended periods of time and for those who are completely new to sobriety, and are attempting the journey towards recovery for the first time.

At The Hope Center for Rehabilitation, we provide highly qualified, experienced case managers to each of our clients, pairing them based on their personalized needs and preferences.

Our case managers work to help clients adjust to life without drugs or alcohol, assisting them with everything from career objectives to educational goals in order to achieve fulfilling, long-term sobriety after treatment.

The Hope Center

The Hope Center for Rehabilitation offers a full range of services both leading up to, during and following treatment, including professional interventions, a luxury, medically assisted detox program, inpatient rehabilitation (30-90 days), intensive out-patient rehabilitation and out-patient services. Each of our clients become a part of our alumni program at the completion of their treatment to help foster a continued community of recovery.
